Improve performance of Dynamic Application Factories

Also added a method overload to provide a Package Name.
Useful for DX based second generation packages. Now each package can have their own bindings.

Trigger Handler functionality
...
A trigger handler based on trigger actions configured via custom metadata. It separates the trigger logic per class, and is utilizing the fflib_UnitOfWork to execute the trigger actions.

Add Dynamic Application Factory
...
This enabled realtime dependency injection for the fflib application.
By default the configuration comes from the Custom Metadata object fflib_AppBinding__mdt, but can be fully customised and extended.
